St. Louis New Patient Forms

Thank you for contacting Associated Hearing Professionals regarding your hearing healthcare needs. Below, you’ll find information about our practice. With over 70 years of combined experience, we provide comprehensive audiology services, including hearing evaluations, new hearing aid fittings, repair, reprogramming, and fine-tuning.

What to Expect at Your Appointment

At your first appointment, your audiologist will complete a review of your hearing health history, a visual examination of your ears, and a comprehensive hearing evaluation. If you are a candidate for hearing instruments, you will be given the opportunity to listen with some in the office. We recommend that you bring a family member or friend with you, as it is helpful to have a familiar voice to listen to when you are trying new hearing devices.

Please review the downloadable brochure, fill out the enclosed patient registration forms, and bring them with you to your first appointment. We also ask our patients to bring their current insurance cards and driver’s licenses. If you do not have a valid license, please bring in the identification card the state provided you.

Payment and Insurance Options

As a courtesy, we will file claims on your behalf with your insurance company. Patients with Medicare, HMOs, or PPOs, please contact your Primary Care Physician (PCP) prior to your visit to obtain your doctor’s order. Your physician needs only to fax a request for a “hearing evaluation” to our office at (636) 778-9236. If your insurance does not pay for office visits or supplies, if you have a copay, or if you are uninsured, we ask that services be paid before leaving the office.

As a courtesy to you, our office accepts cash, personal checks, and credit cards. We work with CareCredit to offer financing for your hearing expenses, and we will be glad to provide you with further information about this.

Our practice strives to exceed your highest expectations by providing the time and individual attention you deserve. At Associated Hearing Professionals, we are truly passionate about improving your quality of life through better hearing. Please feel free to contact us with questions at (636) 778-9232 or via email at mail@HearSTL.com.

Again, thank you for contacting Associated Hearing Professionals. We look forward to meeting you.
